# Corrective Action Report: Open It From the Form

## How do you open a corrective-action record for a defect?

To open a corrective-action record for a defect, run it when a nonconformance form is submitted. Maito links the lot and order, finds other lots from the same setup, requests a hold in your inventory system for approval, assigns the investigation, and posts the record in Slack. It leaves the disposition fields empty.

**Maito** (1:52 PM):

**Defect:** bore oversize on 12 of 40 sampled pieces, lot L-5821-B  
**Stock:** 160 pieces in lot, 40 in finished goods, 120 at deburr

Hold requested for L-5821-B in the inventory system. It's waiting for Mina's approval.

**Catch:** lot L-5821-A, 100 pieces, ran on the same setup and shipped Oct 15. It has no sample records. I've listed it as possibly affected.

Investigation task assigned to Raul, due Oct 22. From the inspection form and the inventory records.

**Mina** (2:01 PM):

Approve the hold on B and add A to the investigation.

**Maito** (2:02 PM):

Hold confirmed on L-5821-B (160 pieces). L-5821-A added as potentially affected. The disposition fields stay empty for you. Want a draft customer notice about A for your review?
